Five expert perspectives.

February 26, 2016

From improving disease management in the elderly and streamlining care transitions to ensuring patients have access to the information and tools they need to manage their health and take control of Empowered Care Sidebartheir treatment journeys, empowered care is as important for healthcare stakeholders as it is for the patients they serve. Five featured experts offer their insight on what empowered care in our communities means to them.

Relative to an aging population…

"Empowered care is when residents feel much more at peace and secure in the knowledge that their community has in place the programs and mechanisms to support their health - both for immediate needs and concerns as well as continued maintenance and wellness. People are living longer and wanting to age in the community they've lived in and loved their entire lives. An empowered patient is someone that knows they can stay in their own home - that because of innovations and unique collaborations in the community, they can continue to access care and live as an independent 85-year-old."

Relative to patient engagement…

"We now have technology that will allow us to be able to connect with patients on a much deeper level than we ever dreamed we could. If we can not only empower community pharmacists with the right tools at the right time for the right purpose, but also empower the patient with those same tools to be able to create an interactive experience that helps them reach those health goals... there's power in that."

David Pope, PharmD, CDE

Relative to medication management…

"While traditional care is linear in its approach to the patient, we are now seeing a more fluid process. Care needs to surround the patient - reacting and proactively engaging, inquiring and interacting with the patient to enlist them as an active participant in their treatment regimen and drive the best clinical outcome."

Matt Wolf
Group Vice President, Consulting Practice
Pharmacy Healthcare Solutions at AmerisourceBergen

Relative to patient-pharmacist relationships…

"Empowered care creates a connection between the patient and pharmacist. For patients, I think it's the understanding that they can go to a pharmacist and ask questions about their health and know the pharmacist will be able to have a meaningful conversation with them. The empowered pharmacist is now taking time away from filling scripts to strengthen those patient relationships. And because of that, pharmacists are seen as a critical component in the continuum of care and as someone who holds an important role within the community."

Relative to communications…

"Empowered care means putting the patient at the center of everything. It's where the medical community is not only providing continuous, coordinated care, but also truly understanding what the patient wants. It's giving the patient control and encouraging them to take it - to be vocal, to be communicative. Because this is a partnership, a journey - one in which we as healthcare providers are privileged to take with them."
